Claims Procedure

Kindly refer to the following step-by-step claims procedure by clicking on the relevant insurance policy.

Make a policy report if there is evidence of criminal or malicious act.

Complete Claim Form and Prepare the documents required :

  • Original Medical Bills and Medical Certificates
  • Copy of Work Permit / Employment Pass (for foreign employees)
  • Copy of Medical Report or Inpatient Discharge Summary
  • Copy of death certificate if the accident resulted in death of employee

Contact your insurance agent and submit the completed claim form

If it is related to theft, burglary and/ or malicious damage :

  • Report to police within 24 hours
  • Take photo of the damage property
  • keep the receipt and / or credit card statement of the damage property

Complete Claim Form and Submit to your insurance agent

  • General Documents Required
  • Copy of Passport
  • Copy of travel itinery / boarding pass
  • List of documents required by Insurance Company

Employers should report any work-related accidents to MOM using iReport System at http://www.mom.gov.sg/iReport, within the time frame below:

What to report Reporting Timeline
Accidents results in death of an employee Within 10 days from the accident happened
Accidents results in the employee unfit for work more than 3 calendar days or hospitalised for at least 24 hours
Employee contracts an occupational disease

Complete claim form, prepare required documents :

  • Copy of iReport submitted to MOM
  • Original Medical Bills and Medical Certificates
  • Copy of Work Permit / Employment Pass (for foreign employees)
  • Copies of  payslips of the injured worker for past 12 months from the accident and during Medical Leave Period
  • Copy of Medical Report or Inpatient Discharge Summary
  • Number of working days per week
  • Copy of death certificate if the accident resulted in death of employee

If injured worker is a sub-contractor’s employee or engaged in a project work, please prepare the documents below:

  • Contract agreement between insured and sub-contractor and/or main contractor
  • Copy of sub-contractor’s and / or main contractor’s workmen compensation policy

Contact your insurance agent and submit the completed claim form

Reporting

Call the ambulance and lodge a police report immediately if it is involved any of the following:

  • Bodily injury
  • Government vehicle or property
  • Foreign vehicle
  • Outside of Singapore
  • Pedestrian or cyclist
  • Hit – and – run driver
  • Vandalism
  • Vehicle caught fire

Obtain information

  • Exchange third party’s particulars including Name, NRIC/Fin Number, Phone Number
  • Third Party’ s vehicle number
  • Take digital photographs of all damage vehicle and accident scene

Report accident

  • Report and bring the accident vehicle to Authorised Workshop or reporting centres within 24 hours or by the next working day.
  • Bring along driver’s driving license and Certificate of Insurance
  • Inform workshop for reporting purpose:-
    • Reporting Only
    • Claiming against own motor insurance
    • Claiming against Third Party’s motor insurance
